Must purchase in person. Must schedule Phlebotomy and Exam/Consultation at the time of purchase.

If a patient is non-handleable (e.g. fractious or aggressive) we reserve the right to refuse service for the protection of the patient and our staff.

If the patient is ill or has evidence of infection (to include: skin, ears, eyes, upper respiratory etc) we reserve the right to refuse service in the best interest of the patient.

If the test results warrant further testing and/or treatment, these additions are not included in this promotion.
